With a sturdy risk management programme in place this might not have happened.......
This is the first of a series which will give ACTUAL losses that could have
been avoided if a sturdy risk management programme was in place, and in many
cases how we have assisted....
Earlier this year, we were called in to assist a client who was having major
construction works in progress on a single construction site. In Fiji we design buildings to withstand cyclones and earthquakes to the Fiji
Building Codes, which in some cases refer to either New Zealand or Australian
standards.
On this site our client engaged Australian consultants, and they, mistakenly,
appeared to design all the works to Australian standards. Only after much of the structural construction work was complete, was this
apparant anomaly discovered.
We were asked to draw up a report that gave a risk management perspective of
the extent of the problem, so as to assist our client's management and board
in deciding whether to accept the design and construction, or whether to effect
design and construction changes costing many tens of thousands of dollars.
The issue here, though, was why was this design anomoly not discovered earlier.
Perhaps this could have been avoided if there had been a really sturdy risk
management programme in place.
